Principles and Clinical Applications of Photopic Negative Response(PhNR)
Abstract
Background : To compare the parameters of photopic negative response (PhNR) among normal, primary open angle glaucoma (POAG) and diabetic retinopathy groups, and to estimate the clinical utility of PhNR.

Methods : Electroretinography (UTAS E-3000®, LKC Technologies, Gaithersberg, MD, USA) was performed for totally 30 subjects, 60 eyes (Normal group, POAG group and DR group). Amplitudes and implicit times of PhNR were compared among groups. Mean deviation (MD) and pattern standard deviation (PSD) were evaluated with standard automated perimetry (SAP), and mean retinal nerve fiber layer (RNFL) thickness and cup-disc ratio (C/D ratio) were measure with optical coherence tomography.

Results : a-wave and b-wave showed no differences among groups. Conversely, amplitudes of PhNR were reduced in POAG and DR groups compared to Normal group (p<0.001). Implicit times of PhNR were prolonged in POAG and DR groups (p<0.001). Parameters of PhNR had correlation with MD and RNFL thickness.

Conclusions : PhNR may be useful to detect the inner retinal dysfunction including glaucoma and diabetic retinopathy.
